Patient

65 yr old male with no medical history presents with "back injury" from lifting weights. 4 days of pain under L scapula. Pain wraps around to front and is worse with movement. No SOB, neck pain, neurologic deficits. Differential diagnosis.

ED Course

Labs, CXR, EKG. EKG shows inferior Q waves with nonspecific ST changes and slight ST depression V5 and 6. Aspirin, O2, NTG given and pain meds. Patient is pain free when troponin comes back elevated.

Cardiology is consulted for a NSTEMI (non ST elevation MI) Cardiac catheterization shows 100% RAD occlusion.
